By checking the above box, I hereby authorize Sing for Hope the right and permission to use, reproduce, and/or publish my photographs, voice, and/or work that may pertain to me-including my image, likeness, voice and/or artwork- without compensation. I understand that this material may be used in various publications public affairs releases, recruitment materials, broadcast public service advertising (PSAs) or for other related endeavors. This material may also appear on Sing for Hope's website or social media pages. This authorization is continuous and may only be withdrawn by me in writing with reasonable advance notice. Consequently, Sing for Hope may publish materials, use my name, photograph, and/or make reference to me in any manner that Sing for Hope or project sponsor deems appropriate in order to promote/publicize service opportunities. Any proceeds that may be made from the use or sale of these materials will be used to support Sing for Hope's ongoing programming. I hereby waive any right that I may have to inspect or approve the finished product or any written copy that may be used in connection therewith.
